Position Summary :

The Intake Coordinator at United Returning Citizens (URC) plays a pivotal role in the intake process for individuals seeking support services as they reintegrate into society. This position involves gathering essential client information, determining the services that are needed, and assigning clients to the appropriate staff member for further support. The Intake Coordinator ensures that clients are efficiently placed into the URC system and receive the necessary services to aid in their successful reintegration.

Key Responsibilities :

I. Client Intake & Assessment :

  • Initial Client Information Gathering :

Conduct intake interviews with clients to gather personal information, background, and specific needs related to reintegration.

  • Input client data into the case management system (Casebook) with accuracy and attention to detail.
  • Review all intake forms and ensure that all necessary information is collected, including contact details, history, and specific service requirements.
  • Service Determination :
  • Assess the clients needs based on the information collected during intake and identify which services from URC will be most beneficial.

  • Collaborate with other staff members to ensure clients are referred to the correct programs (e.g., housing, employment, legal assistance, mental health support).
  • II. Case Assignment & Coordination :

  • Assigning Clients to Appropriate Staff :
  • Based on the intake assessment, assign clients to the appropriate URC employee or program for follow-up services.

  • Ensure clear communication with relevant staff members regarding client needs and case details.
  • Service Tracking :
  • Track and monitor the progress of clients intake and ensure they relate to services in a timely manner.

  • Follow up with clients to ensure they are receiving the services they were assigned.
  • III. Client Support & Guidance :

  • Providing Information :
  • Inform clients of the services URC offers and help them understand the intake process and what to expect next.

  • Address any questions or concerns clients may have regarding the intake process.
  • Connecting Clients to Resources :
  • Provide clients with information about additional community resources or services they may need during their reintegration.

    IV. Documentation & Data Management :

  • Accurate Record Keeping :
  • Maintain accurate and up-to-date records of all client interactions, assessments, and service assignments in the case management system.

  • Ensure that all documentation is filed and stored according to URCs privacy and confidentiality guidelines.
  • Reporting & Data Analysis :
  • Prepare periodic reports regarding intake activity, including client numbers, services assigned, and any trends observed.

  • Help analyze intake data to identify potential areas for improvement in client services.
  • V. Administrative Support :

  • Supporting Intake Workflow :
  • Assist with managing the flow of new clients into URCs programs and ensure smooth transitions for each client.

  • Perform clerical duties related to client intake, including filing, answering phones, and responding to emails related to intake processes.
  • VI. Collaboration & Communication :

  • Team Collaboration :
  • Work closely with other URC staff, including case managers, program directors, and service providers, to ensure effective client service coordination.

  • Maintain communication with all stakeholders involved in the clients care to facilitate coordinated support.
  • Community Networking :
  • Maintain knowledge of local resources and services that may be beneficial to URC clients.

  • Participate in networking opportunities to learn about additional services and referrals that can enhance client care.
  • Qualifications & Skills :

    Experience :

  • At least 1-2 years of experience in case management, intake coordination, or related fields, preferably within social services or non-profit settings.
  • Experience with case management software or databases, such as Casebook, is preferred.
  • Communication Skills :

  • Strong verbal and written communication skills.
  • Ability to communicate clearly and effectively with clients, staff, and community partners.
  • Organizational Skills :

  • Excellent organizational skills with the ability to manage multiple cases and maintain attention to detail.
  • Ability to prioritize tasks effectively in a fast-paced environment.
  • Empathy & Cultural Competence :

  • Ability to work with individuals from diverse backgrounds, particularly those who have experienced incarceration or other barriers to reintegration.
  • Empathy and understanding of the challenges faced by returning citizens and a commitment to supporting their success.
  • Problem-Solving & Critical Thinking :

  • Strong problem-solving abilities and critical thinking skills to assess client needs and determine the best course of action.
  • Ability to be flexible and adaptable when handling client cases and service needs.
  • Work Environment :

  • Ability to work both independently and as part of a team in a dynamic, client-centered environment.
  • Transportation & License :

  • Reliable personal transportation and valid drivers license required for site visits, meetings, and outreach efforts.
  • Additional Requirements :

  • Ability to handle sensitive and confidential information with professionalism and discretion.
  • Must be flexible to work outside standard office hours when necessary, including evenings and weekends, to accommodate client needs.
