About the Director of Advanced Practice Providers (APPs)
Upperline Health is seeking a leader for our team of Advanced Practice Providers to report directly to the Chief Medical Officer and President of Value Based Care. This leader will manage strategy and operations for a growing team of APPs across 19 states. The APP team at Upperline Health partners with our employed specialists to provide longitudinal care to manage patients’ chronic diseases, as well as acute care for urgent issues – this includes seeing patients in the home, via telehealth, and in our clinics.
The APP Director will work in collaboration with Upperline’s medical directors and interdisciplinary care team (including nurses, social workers, pharmacists, dieticians, and care managers) to offer an excellent patient experience, with the goal of improving patient outcomes and keeping patients out of the hospital. The APP Director will oversee all day-to-day operations, including scheduling (daily and call schedules), urgent patient issues, organizing interdisciplinary team meetings, and performance management. The APP Director will also oversee hiring, onboarding, training, and continuing education for APPs.
This role is an opportunity to join a growing and innovative team as the operational leader of the APP team, and to work closely with medical directors to ensure high quality clinical training and clinical care for patients. Ideal candidates will have experience working in value-based care and with Medicare patients. Most importantly, candidates must demonstrate compassion for patient needs, have great attention to detail and thrive in a collaborative environment.
