Responsibilities
The Community Health Worker :
- Educates the community about healthy living, and encourages the population to get regular screenings
- Coordinates health education activities under the supervision of the Community Health Worker Supervisor or Health Education Manager as needed.
- Discusses health concerns with the community members.
- Conducts outreach programs and workshops which may require working nights and weekends.
- Facilitates access to healthcare services; provides ongoing support and services as needed, to include participating in care coordination and / or case management, as well as providing referrals to social services agencies and subsequent follow-up.
- Plans work in consultation with CHW Supervisor and relevant members of the multi-disciplinary care delivery team, to include obtaining proper materials within the necessary time frame.
- Opens and maintains communication with local healthcare providers.
- Advocates for the needs of the individuals and community they serve to include opening and maintaining communication with local healthcare providers.
- Organizes local outreach programs to raise awareness about the existence of the initiative, making it possible for everyone to access the services needed
- Collects health data and program data from a population to report to CHW Supervisor.
- Coordinates educational functions with those of other public health programs; provides health coaching and supports self-management of disease prevention as well as of health conditions, including chronic diseases.
- Participates in program evaluation and research as required.
- Regular and prompt attendance.
- Ability to work schedule as defined and overtime as required.
- Related duties as assigned.
Qualifications
High school diploma and completion of a National Standard C3 aligned Community Health Worker training program -OR - High school diploma, two years of related community-based experience and enrollment in a C3 aligned Community Health Worker training program. Higher education / related healthcare training program may substitute on a year-for-year basis for the required experience.
