What are the responsibilities and job description for the Community Health Nurse position at Ute Mountain Ute Tribe?
Summary: The Community Health Nurse provides continuity of care for tribal and non-tribal clients on the Ute Mountain Ute Reservation in Towaoc, Colorado and White Mesa, Utah by making client contacts to ensure compliance with medication, diet, exercise, dialysis, and provide risk assessment for safety and prevention of infectious disease in both Towaoc and White Mesa communities. The Team Member will work closely with the I.H.S. clinic staff in the field setting, providing education, training, support and follow-up care.
Minimum Qualifications:
High School Diploma or GED.
Must have completed a nursing program from an accredited college or university.
Must hold a current Colorado Nursing Licensure without encumbrance from the Colorado State Board of Nursing or compact state.
A minimum of three years in home health nursing or related field is preferred.
Must be able to pass a background check.
Valid driver's license, and current First Aid and CPR certification.
