VCSB is seeking an ACT Nurse/Case Manager! The ACT Nurse/Case Manager will provide intensive case management, rehabilitation, and supportive services to individuals experiencing serious mental illness.

Summary of Essential Functions
- Serves as primary provider of Assertive Community Treatment (ACT).
- Provides nursing and case management services to individuals experiencing chronic mental illness.
- Assists individuals in maintaining independent living.
- Meets requirements for documentation of client service delivery and maintains records in accordance with agency and regulatory standards.
- Provides clinical and program consultation to individuals, organizations, agencies, and groups that are involved with VCSB.
- Participates in team meetings, clinical staffing, and supervision.
- Participates in in-service training/staff development activities regarding the operation of ACT services.
Minimum Qualifications
All applicants must meet the following minimum qualifications in order to be considered qualified for the position:
- Associate’s Degree in Human Services,
- Active RN or LPN license,
- QPPMH, QMHP, or QMHP-T eligible,
- One year of clinical experience working with adults experiencing chronic mental illness, and
- Valid Virginia driver’s license with acceptable driving record
