We are seeking talented Full-Time Care Coordinators.
As a Certified Peer Support Worker, you will provide direct patient care to patients under the supervision of our licensed professionals while maintaining a safe and efficient working and treatment environment. Your ability to collaborate effectively with clinicians and multidisciplinary team members to ensure safe quality patient care will be essential to success.
POSITION SUMMARY
- Under the supervision of clinical, provide direct patient care to patients as assigned.
- Carry out all aspects of patient care and other assignments as directed by the Clinical Director
- Observe patients’ behavior
- Perform ongoing observation of the patients and milieu to ensure active treatment is provided and patients are participating.
- Respond appropriately and timely to patient requests.
- Report changes in patients’ condition or behavior to clinical
- Intervene appropriately with patients experiencing a behavioral crisis situation
- Report factors that may impede successful accomplishment of the treatment plan to the Charge Clinical.
- Provide patient educational information for identified learning needs.
- Facilitate educational/activity groups as assigned or when indicated using approved materials.
- Monitor vital signs such as heart rate and blood pressure
Responsibilities
- Provide crisis management and intervention services to clients in need.
- Assist clients in developing coping strategies and life skills through mentoring.
- Conduct assessments to identify client needs and create personalized support plans.
- Facilitate group sessions and individual counseling focused on behavioral health and mental health issues.
- Utilize motivational interviewing techniques to encourage clients towards positive change.
- Collaborate with multidisciplinary teams to ensure comprehensive care for clients.
- Maintain accurate documentation of client progress and interventions in accordance with organizational policies.
- Advocate for clients’ needs within the community and connect them with appropriate resources.
Skills
- Strong crisis management and crisis intervention abilities.
- Experience in mentoring individuals from diverse backgrounds.
- Knowledge of behavioral health principles and practices.
- Proficiency in mental health counseling techniques.
- Familiarity with addiction counseling methods and systems.
