Title: SSVF Case Manager – Shallow Subsidy
Job Purpose/Description:
A full-time position providing case management services and supports to veterans in the SSVF program. Flexible hours, including evenings and weekends and an ability to travel within the service delivery areas are required.
Reports To:
Veterans Services of the Carolinas Assistant Director of Housing
Job Responsibilities:
- Provides eligibility screening to applicants and collects and maintains accurate and detailed records.
- Coordinates and monitors all aspects of service delivery including intake, assessment, recommendations on housing stabilization, treatment planning, budgeting, case conferences, referrals for adjunctive services, family meetings, group work, individual counseling, advocacy, and liaison with other community agencies.
- Ensures all participants are served as required by the grant guidelines.
- Provide shallow subsidy case management services to at low income Veteran participants consistently throughout a 2-year period.
- Develop, implement and revise 90-day and 2-year housing stability plan with Veteran participants.
- Documents service delivery and maintains accurate, timely documentation and is responsible for client confidentiality.
- Ensures accuracy and consistency with the agencies fiscal and billing procedures.
- Transports veterans in a safe, cautious and responsible manner.
- Builds positive working relationships with property providers that are engaged in serving the population.
- Participates in all appropriate staff, supervision, and training meetings.
- Assumes other duties as assigned by the Director.
Qualifications:
- Bachelor’s Degree in a Human Services-related field preferred.
- Combination of work and/or relevant work experience will be considered in lieu of degree requirements.
- 3 – 5 years of human service related experience required.
- Experience working with veterans preferred.
- Must possess a valid driver’s license and proof of insurance, with a good driving record.
- Must have knowledge of clinical issues, skill in community resource development, be well organized, a self-starter, and able to work as a member of a team.
Job Type: Full-time
Pay: $38,000.00 - $42,000.00 per year
