Overview

The VNA Health Group-Moving Healthcare Forward

The VNA Health Group is seeking a Medical Social Worker for Essex County with availability to cover additional counties such as Hudson, Union, Passaic and Morris as needed. This position is responsible to provide psychotherapeutic counseling and support to End of Life care for hospice patients and families, with a particular emphasis on those experiencing psychosocial and/or interpersonal challenges.

Responsibilities

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Develops and implements a social work plan of care
  • Collaborates with professional staff on psychosocial problems and appropriate interventions
  • Provides patients and families with information on community resource options and assists in linking with needed resources
  • Adheres to the organization’s policy and procedure on documentation
  • Adheres to agency infection control and safety policies
  • Adheres to the organization’s policy in regards to absenteeism and appearance
  • Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

Qualifications

  • Masters Degree in Social Work required with current NJ LSW
  • 1 year of supervised clinical experience; community health experience preferred
  • Excellent verbal and written communication skills
  • Excellent interpersonal skills
  • Ability to work with multiethnic and diverse socioeconomic populations

Working Conditions/Physical Demand : Services are provided in the patient’s home in a community or facility setting
