Key Responsibilities

• Deliver personalized one-on-one care to clients in their homes.
• Conduct vital signs assessments and physical evaluations.
• Administer prescribed medications and treatments.
• Maintain comprehensive documentation of patient conditions.
• Provide specialized care for G-tubes, tracheostomies, and ventilators as required.
  
  

Required Skills

• Excellent patient interaction and communication abilities.
• Competence in performing vital signs and physical assessments.
• Proficiency in administering medications and treatments as directed by physicians.
• Strong documentation skills to track ongoing patient health status.
• Experience in G-tube, trach, and ventilator care is advantageous but not mandatory.
  
  

Qualifications

• Valid nursing license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
• Pediatric experience is preferred but not essential.
