Description
Immediate full time position for a Peer Behavioral Health Coordinator. This is a new position and will be stationed at the TTU campus in Cookeville.
The Peer Behavioral Health Coordinator is expected to follow guidelines and maintain a level of performance consistent with current grant guidelines.
General Characteristics of Duties:
Provide integrated care management services to young adults with Serious Emotional Disturbances (SED) and co-occurring physical health or substance use conditions. Focus will be on promotion, intervention, and recovery support services by one of the following services:
1. – Initiate, complete, update, and monitor the progress of a comprehensive person-centered care plan (as needed).
2. – Participate in the patient’s physical health treatment plan as developed by their primary care provider as necessary. Support scheduling and reduce barriers to adherence for medical and behavioral health appointments. Proactive outreach and follow up with primary care and behavioral health providers.
3. – Identify and facilitate access to community supports (food, shelter, clothing, employment, legal, entitlements and all other resources). Communicate patient needs to community partners. Provide information and assistance in accessing services.
4. – Provide high-touch in-person support to ensure treatment and medication adherence. Provide caregiver counseling and training. Identify resources to assist individuals and family supporters
5. – Provide additional high touch support in crisis situations. Participate in development of discharge plan for each hospitalization. Develop a systemic protocol to assure timely access to follow-up care post discharge. Establish relationships with other treatment settings. Communicate and provide education.
6. – Education the patient and his/her family on independent living skills with attainable increasingly aspirational goals.

- Assist clients in working with their treatment team in determining the steps he/she needs to take to achieve these goals and self-directed recovery.
- Assist clients in setting up and sustaining self-help (mutual support) groups, as well as means of locating and joining existing groups.
- Utilize relapse prevention techniques to assist clients in creating their own individual wellness and recovery plans.
- Models effective coping techniques, self-help, and recovery resilience strategies.
- Teach problem-solving techniques with individual clients and groups; engage clients in discussions sharing common problems in daily living and methods employed to manage and cope with these problems.
- Will share their own life experiences and what skills, strengths, supports and resources they use to continue their recovery journey speaking as peers who have availed themselves to addiction and/or mental health services.
- Attend meetings and trainings in-house, electronic, virtual, etc. as directed by supervisor
- Engage assigned clients in fun, recovery activities when appropriate to aid in emotion regulation, behavior modification and redirection as needed.
- Assist clients in building social skills in the community that will enhance employment skills, resume, and job searches when needed.
- Monitor, educate and assist clients in performance of activities of daily living in accordance with goals and problem areas identified on individualized treatment plans.
Qualifications
Must have a Bachelor’s degree in a health-related field of counseling, psychology, social work or other behavioral sciences. Certified Young Adult Peer Support Specialist through TDMHSAS or must be working toward CYAPS and obtain phase 1 certification within first six months of employment.
Salary : $38,000 - $42,000
