Shift Details: Full-time – 40 hours/week
Compensation: $50,000-$53,000 annual salary

About the job:
The primary responsibility of the Residential Case Manager is to provide treatment-based case management services to assure each resident’s needs are met and the individual treatment plans are adhered to by the treatment team.
Essentials:
- Act as the primary point of contact for external care team, from scheduling admissions and discharges to post-discharge and aftercare follow up.
- Ensure all intake and discharge paperwork and tasks are complete and documented as assigned.
- Facilitate individual and group skills.
- Provide leadership and support to milieu team including consultation and guidance for milieu interventions.
- Assist milieu teams in behavior management, crisis intervention, and restrictive procedures as needed.
- Complete assessments and develop trauma-informed interventions in collaboration with treatment team.
- Coordinate with external providers as requested and/or needed for ongoing treatment/service updates and needs.
- Complete insurance re-authorizations/alternate funding as needed to ensure funding for assigned residents.
- Meet affirmative action goals established by the organization while maintaining a work environment which respects and supports cultural awareness and proficiency.
- Participate in administrative support rotation to provide guidance via phone for direct care staff.
- Communicate effectively, both orally and in writing, with care team, external providers and placing authorities.
- Demonstrate ability to work independently, prioritize tasks, and manage time effectively.
- Represent program and VOA agency professionally and ethically.
- Conduct intakes and complete necessary documentation and assessments for each youth.
- Maintain working knowledge of DHS Rule 2960 to ensure DHS standards and regulations are met.
- Facilitate discharge planning and ensure discharge paperwork is complete and filed.
- Attend required weekly meetings.

Required Qualifications:
- Bachelor's degree in Human Services related field (e.g. Social Work, Psychology, Counseling) OR equivalent work experience ideally with children / adolescents experiencing mental, emotional and/or behavioral disturbances
- Case Management experience preferred and not required
- Ability to think critically and work independently
- Must be 21 year of age or older
- Must pass required background check
