We are seeking a dedicated Care Manager to join our team. The ideal candidate will have a passion for helping others and possess a diverse skill set in areas such as motivational interviewing, childcare, addiction counseling, behavioral health, crisis intervention, Medicare knowledge, supervising, workforce development, crisis management, and Section 8 assistance.
Essential Functions:
- Provide comprehensive care management services to clients in need
- Conduct assessments to identify client needs and develop individualized care plans
- Utilize motivational interviewing techniques to engage clients in their care
- Coordinate with external agencies and resources to support client needs
- Offer addiction counseling and behavioral health support as needed
- Supervise and mentor staff members to ensure quality service delivery
- Assist clients in navigating Medicare benefits and entitlements
- Implement crisis intervention strategies when necessary Oversee workforce development initiatives for client empowerment
- Provide guidance on Section 8 housing options for eligible individuals
Education and Experience Requirements:
- High School Diploma or GED
- Certified Addiction Tech (CAT) or LPCC/LSW/MFTC
- Proficiency in motivational interviewing techniques
- Knowledge of addiction counseling principles
- Strong background in behavioral health services
- Ability to handle crisis intervention situations effectively
- Familiarity with Medicare regulations and benefits
- Experience in workforce development programs
- Proficient in crisis management strategies
- Understanding of Section 8 housing programs
