Position Details

Applications must be submitted by January 23rd. 

 

Under direction, performs work in providing social work services on cases involving complex social problems and social treatment plans; performs responsible and difficult casework, directs service programs, or provides work direction to paraprofessional staff; and performs related work as assigned.
 

Conducts MnChoices Assessments

  •      Contacts client, assess need for MnChoices assessment.
      • Interacts with Intake and HCBS Supervisor to ascertain criteria for MnChoices Assessments.
      • After initial MNChoices assessment completes assessment summary within required timeline.
      • Along with the HCBS Unit Supervisor serves as a MnChoices program mentor and internal trainer.
      • Tracks assignments related to timelines, progress and completion of all MnChoices Assessments assigned to your caseload.
    •      Completes MnChoices assessments within required timeline.
    •      After initial MnChoices assessment completes Assessment Summary within required timeline.
    •     Develops Assessment Summaries that reflect the person-centered principles outlined by DHS.
    •     Makes appropriate referrals for identified supportive services as needed.
    •     Coordinates service initiation (as appropriate) with case managers (if any) assigned to continuing care.
    •     Brings comprehensive information regarding completed assessments to the review team to determine eligibility for waiver funded programs or case management.
    •     Mails all required documents to the person who the assessor completed the assessment with (i.e., Assessment Summary).
    •    Completes other duties as assigned, including, intake, crisis intervention to facilitate early treatment and rehabilitation, and education for various target populations and the general public.  
      • Provides information and referral to persons contacting our agency.
      • Provides crisis intervention coordination and problem identification to persons requesting and other community agencies, if possible.
    •     Completes initial assessments for Community First Services and Supports (CFSS). Assessor will complete the support plan, send referrals to the consultation provider as well as collaborate with public health on entering the screening document into MMIS for approval.

Additional Duties and Responsibilities

  • Attends training/workshops outside agency as applicable/available.
  • Completes DHS web-based training when appropriate.
  • Participates in annual individual evaluations.
  • Answers co-worker questions: consults with co-workers when needed.
  • Reads routed material in a timely manner.
  • Researches manual materials; on-line materials.
  • Seeks help from DHS and as needed.
  • Participates in training for personal and professional growth.
  • Attends and participates in staff meetings.
  • Attends agency meetings.
  • Attends Board meetings when requested.
  • Assists in completing agency reports.
  • Participates in agency reorganization (files, procedures, etc.) as requested.
  • Participates in community public relations efforts when requested.
  • Completes Random Moment form.
  • Performs other duties as assigned by supervisor.
  • Completes other duties as assigned, including, intake, crisis intervention to facilitate early treatment and rehabilitation and education for various target populations and the general public.
  • Provides information and referral to persons contacting our agency.

 

While these are the primary focus of the position, we believe strongly in teamwork and employees will be called upon to perform a variety of duties as part of their role with the County.

  • Knowledge of the principles and techniques of interviewing and recording in social work and the ability to apply them.
  • Knowledge of individual and group behavior.
  • Knowledge of social casework objectives, principles, and methods.
  • Knowledge of social-economic factors, which promote stable family life and understanding of the elements, which affect family security.
  • Knowledge of the principles of community organization.
  • Knowledge of physical and mental illnesses.
  • Knowledge of social welfare research methods.
  • Knowledge of family systems and dynamics.
  • Ability to effectively apply casework knowledge and skills.
  • Working ability to work constructively within an agency in the community setting and in effectively utilizing appropriate resources and services.
  • Ability to work constructively in the development and coordination of community resources to meet special needs.
  • Ability to manage time productively.
  • Ability to prioritize caseloads.
  • Ability to communicate effectively, both orally and in writing.
  •     Ability to work under limited supervision with general autonomy in determining how objectives are achieved; supervisors generally set operating benchmarks, goals, and objectives.
  •     Ability to apply procedures, policies and/or precedents requiring analytical ability in adapting standard methods to fit facts and conditions using generally prescribed procedures.
  •     Ability to maintain effective communication and working relationships with numerous outside and inside contacts to achieve work objectives.

Education and Experience

  • Bachelor’s degree from accredited university/college with a major in Social Work or other approved social services degree.
  • Education and experience/background as required by Rule 79 or MN Rules 9520.0900 to 9520.0926.
  • Equivalent combination of education and experience.
  • Valid MN driver’s license required.

Physical and Mental Requirements

This position requires light physical duties often associated with office positions. Employee must be physically able to perform the essential job functions of this position including the ability to sit, stand, reach by extending arms, occasionally lift and/or move twenty pounds unassisted, have unrestricted use of the upper body, be able to stoop, kneel or crouch, have corrected or uncorrected hearing and vision within normal ranges, and a clear speaking voice. Noise levels within the workplace are described as moderate. Reasonable accommodations may, at times, be made to enable individuals with qualified disabilities to perform the essential functions of this position. There is prolonged exposure to computer keyboards and video screens. This position is generally light-duty and may require the exertion up to 20 pounds of force.
