HiYH delivers hospital-level care to patients at home, and one of its key programs, focuses on managing chronic conditions through personalized support. As an enrollment coordinator for Remote Patient Monitoring (RPM), your role is critical in helping patients manage conditions like hypertension, diabetes, heart disease, and COPD.
With RPM, patients use easy-to-use devices like blood pressure cuffs, glucose monitors, scales, and pulse oximeters to track their health from home. These devices send real-time data to our care team, allowing us to monitor the patient's condition closely and step in if necessary. Your job is to ensure patients are properly enrolled, and our team will ensure they have their devices, and understand how to use them, so we can keep them healthy, independent, and reduce hospital visits.
Patients will receive devices within 3-5 business days.
Patients must use either everyday or every other day.

Pays once per month -

Pay: $814.00 - $938.00 per week
