Overview
We are seeking a dedicated and compassionate Mental Health Targeted Case Manager to join our team. This role is essential in providing support and guidance to individuals facing mental health challenges. The ideal candidate will possess a strong background in patient care and case management, with knowledge of a wide range of a variety of resources and services. You will work collaboratively with clients, families, and healthcare providers to develop comprehensive care plans that connect clients with resources to promote recovery and well-being. This position requires travel from a base in Madisonville, Kentucky, and also serves our Kuttawa, Kentucky, office. Client encounters/visits are anticipated in the Western Kentucky region.
Responsibilities
- Conduct thorough assessments of clients' mental health needs and develop individualized care plans.
- Provide ongoing case targeted management services, including monitoring client progress and adjusting care plans as necessary.
- Collaborate with multidisciplinary teams to ensure holistic treatment approaches.
- Facilitate access to community resources, support services, and treatment options.
- Offer support for clients dealing with addiction or other mental health issues.
- Maintain accurate, timely, and quality documentation of assessments, client interactions, treatment progress, and outcomes.
- Educate clients and families about mental health conditions, treatment options, and coping strategies.
- Advocate for clients' needs within healthcare systems and community resources.
Requirements
- Bachelor’s degree in psychology, social work, education, nursing, or a related field; Master’s degree preferred.
- 1 year experience in mental health service coordination or linking/referring for community based mental health disorders or multi-agency involvement; 5 years experience preferred.
- Familiarity with case management principles and practices.
- Knowledge of pediatric mental health issues is a plus.
- Previous experience with a wide variety of case coordination is beneficial.
- Driver's License and reliable transportation.
- Must complete the case management certification program(s) offered and approved by the DMHMR within 6 months of employment.
- Strong communication skills and the ability to build rapport with clients from diverse backgrounds.
