Job Overview:

We are seeking a high energy and passionate Resource/Community Integration Coordinator.  The ideal candidate will join us in our amazing mission of empowering the lives of adults with intellectual and developmental disabilities (STARS). We are looking for someone with patience, high energy, and excellent problem-solving and communication skills to join our team.  One must enjoy working in an innovative atmosphere and have the capacity to be very flexible in our fast-paced environment.

This position is a dual role with responsibilities in both Resource Coordination and Community Integration.  The role of the Resource Coordinator is to maintain a holistic picture/view of the needs of individual STARS and serve as a connector among all teams/stakeholders to meet the needs of the STARS. This should be done throughout our programming, including service plan management within our databases and the Case Management Agency (CMA).  The role of the Community Integration Coordinator is to coordinate program classes with external community partners.  The Resource and Community Integration Coordinator embraces the Mission Statement of Wellspring Community:  To provide a work, enrichment, and educational community where adults with intellectual and developmental disabilities are empowered to live full, productive, and satisfying lives in a God-centered and nurturing environment.

Resource Coordinator Essential Duties & Responsibilities:

Including but not limited to the following:

• 
  Manages all aspects of case management/resource coordination for approximately 35 day-program participants.
• 
  Collaborates with interdisciplinary teams including Case Managers, Wellspring professionals, parents/guardians, therapists/physicians, and behavioral/speech specialists.
  
• 
  Attends team meetings with case managers for annual service plan renewals, interdisciplinary team (IDT) meetings, SIS level reviews, and other necessary meetings involving STAR support.
• 
  Communicates with private pay families, individual case managers & CMAs (Case Management Agencies)
• 
  Review and process paperwork including monitoring reports, reviewing general IDT notes, and annual review of service plans.
• 
  Analyze progress, coordinate assessments, and develop face sheets.
• 
  Create Individual Service and Support Plans (ISSP’s) for STARS.

Community Integration Coordinator Essential Duties & Responsibilities:

Including but not limited to the following:

• 
  Collaborate with program team to coordinate outings, activities, and events designed to promote socialization, skill development and community engagement.
• 
  Collaborate with clients, families, program staff, and community partners to identify resources and opportunities for community participation.
• 
  Conduct tours of Wellspring Day Program facilities for interested clients and their families, highlighting program offerings, amenities, and community integration opportunities.
• 
  Provide information about our services, philosophy, and approach to supporting adults with intellectual and developmental disabilities.

  Able to work well with team members in all departments within Wellspring Community and the public at large.

  Well-developed communication skills, both written and verbal.
• 
  Proven ability to manage projects, solve problems and take initiative.
• 
  Demonstrates ability to manage challenging behaviors through the execution of positive behavior supports for each individual/unique situation or seeks help from senior positions when necessary.      
  
• 
  Excellent time management skills and ability to multi-task.

Qualifications & Skills:

• 
  High School Diploma or Equivalent required.
• 
  Bachelor’s degree in social work or related field preferred.
• 
  Proven experience in case management, social services, or a related field is preferred.
• 
  Proficiency in computer skills including Microsoft Office and Electronic Charting Systems.
• 
  Must be able to pass a background check and drug screen.
