Job Summary
Substance Use Disorder Case Manager - responsible for screening, assessment and referral.
Responsibilities and Duties
* Responsible for screening services in order to identify the presence or absence of alcohol or other drug (AOD) use where data is collected on an individual in order to make an initial determination if a problem exists and/or to determine if referrals for emergent needs are warranted.
* Responsible for assessment which includes collecting information to ascertain the degree and severity of alcohol and other drug use, as well as to identify the social, physical, and psychological effects of that use.
* Responsible for placement, which includes determining the appropriate level of care, type of service, length of stay and the most appropriate facility for a client utilizing ASAM criteria.
* Responsible for continued stay reviews, which includes reviewing the appropriateness of continued stay at a level of care and/or referral to a more appropriate level of care.
* Responsible for determination of eligibility for treatment services, which is the process of determining what type of service a client is eligible for based on their financial situation.
* Responsible for financial liability, which is the process of determining the amount of treatment costs a client is responsible for, if any.
* Responsible for requesting funding authorizations, which are completed on clients in all levels of treatment.
* Interfaces with BH-MCO care management staff in the placement of Health Choices clients.
* Provides assessment and level of care determinations for Health Choices clients.
* Participates in joint consultation/review of complex cases with BH-MCO care management staff.
* The Case Manager will work in cooperation with social service agencies and community resources in planning to meet the needs of clients and assisting clients with utilizing these resources.
* Responsible for interpreting the program, policies, and procedures to all clients.
* Adhere to the confidentiality practices of the West Branch Drug & Alcohol Abuse Commission, to insure complete discretion of employee, client and visitors rights.
Qualifications and Skills
Four year degree in Human Service related field.
