What are the responsibilities and job description for the NEST Case Manager position at White Bird Clinic?
Position Summary:
As a NEST Case Manager, you are responsible for carrying a caseload of unhoused clients, working walk-ins, meeting clients in the field when needed, collaborating with outside agencies, documenting all clients in the EHR and Service Point, and providing a trauma-informed environment for clients receiving services.
Position Responsibilities
Carry a client load of unhoused individuals, and provide necessary assistance, advocacy, and appropriate referrals.
Perform client assessments and assist with goal development.
Outreach to unhoused clients.
Help clients engage with community resources.
Track and record statistics for required reports.
Attend program and clinic meetings.
Participate in White Bird's Basic Crisis Intervention course, as availability permits.
Help clients engage with community resources.
Required Qualifications
At least two years of experience in a crisis intervention, mental health, substance abuse, or corrections environment
Ability to work comfortably and effectively with diverse populations, especially the unhoused.
Ability to network with other agencies and assist with government resources.
Ability to work on the street and in other non-traditional environments, as well as in an office setting.
Possess a valid Oregon Driver's license and the ability to be insured under White Bird Clinic's vehicle insurance.
Complete White Bird's New Bird Training.
Complete Outreach and Enrollment training and SOAR training within the first 6 months.

Compensation Details:
This is a non-exempt position with a target starting base compensation of $18- per hour paid on a bi-weekly basis.
This position requires successful passage of a State mandated background check.
