Position Summary :
To support and assist the case management staff in their daily workflow, including but not limited to performing administrative tasks such as referral portal management, post-acute approval paperwork, phone, email messages and scanning. Completing calls and email requests for post-acute authorizations, timely communicating information to the case manager or social work to assist in expediting discharges. Delivery of patient choice lists and letters, and assisting with Oxygen orders and communication with our oxygen vendor.
Position Requirements :
Formal Education / Training :
High School Diploma or Equivalent
Workplace Experience :
Prior experience in healthcare
Equipment and Skills Training :
Ability to use multi-line telephone, copier, printer, scanner, use of computer portal and Meditech.
Physical Environment :
Office environment and clinical areas.

Monitors post-acute portal, emails and phone messages timely delivering to staff within 1 hour of receiving information.
Maintains post-acute portal in the delivery of patient information to appropriate post-acute providers as directed by the social worker / Director.
Remains in compliance with departmental delivery of post-acute choice lists to those patients who expect to discharge to HH or SNF.
Manages calls / visits from oxygen vendor and assists with departmental inventory and completion of oxygen paperwork as directed by social worker / director.
Hourly follow up with post-acute portal and daily follow-up with those post-acute providers who have not responded in the portal.
CAHPS standard performance related to “Nurse treat with courtesy / respect” in reference to delivery of Provider choice lists.
CAHPS standard performance related to : “care transitions”.
Maintains positive relationship with post-acute providers, communicating with respect.
Maintains a current ACO list for social workers timely communicating patients’ participation prior to MDR each day.
Monitors and monthly reports social work stats for referrals, preference forms, and other reports as directed by Director.
Preforms other duties within the department at the request of Social Worker / Director.
