What are the responsibilities and job description for the Oncology Patient Navigator/Clinic Coordinator - Cancer Pavilion position at Woman's Hospital?
The Oncology Patient Navigator/Clinic Coordinator supports patients across the cancer care continuum, helping overcome healthcare barriers as part of a multidisciplinary team to optimize care and outcomes. They assist the Navigation/Support Team with administrative tasks, manage department calendars, meeting agendas, and records, and prepare reports and presentations. They also coordinate patient support and survivorship programs and perform other duties per Woman’s Hospital’s policies and mission.
Requirements:
- Requires an undergraduate degree in a healthcare-related field.
- Oncology-specific training and certification (Certified Oncology Patient Navigator- OPN-CG) required or obtained within 3 years of employment.
- Requires three to five years' clerical or administrative experience.
- Medical and/or oncology experience preferred.
Responsibilities:
- Performs various clerical/administrative duties to support team operations
- Daily review of pathology reports for positive cancer diagnosis as well as maintenance of patient spreadsheet to include demographics and care plan information.
- Assists team in planning and coordination of events, meetings, and workshops to include scheduling, design/ flyer distribution, catering requests, and ordering/printing of any educational information.
- Screens patients utilizing evidenced-based screening tools to identify distress, barriers to care, and provides appropriate resources/referrals. Refers to clinical navigators and clinical social workers for further assessment and information as needed.
- Maintains a comprehensive, up-to-date list of resources available to patients, including financial assistance, local support groups, and other relevant community resources.
- Tracks, reports, and evaluates department grants and assistance programs. Ensures all encounters, form data, and patient outcomes are logged in a timely manner in accordance with grant requirements. Creates budget/fund request applications through hospital approved system.
- Coordinates ingoing and outgoing correspondence include faxing, mailing, and scanning of completed financial aid applications, referrals, care plans, distress tools, and letters.
- Inputs data on spreadsheets or other systems/EMR. Documentation of assistance or services provided.
