General Function
The Program Case manager is responsible for making sure the program participants are meeting their goals, accessing the 14 elements and engaging in activities that will lead to participants positively exiting the program. The Program Case Manager supports the YMCA’s Teen Achievers programming initiative by serving as a lead or co-facilitator at middle/high school Achievers at the in-school sites, after school, evening and some Saturday workshops.
Education/Experience/Training/Certifications
Bachelor’s Degree in social work or a related area of work is required, BSW preferred. Any state requirements as needed.
Case management in a youth development program, preferred.
Demonstrated knowledge and experience in working with diverse communities and organizations required.
Reliable transportation is essential. Must possess a competent working knowledge of technology, including Word Processing and a variety of database software required.
Familiarity with OSMIS/ WIOA requirements and systems preferred
Current CPR and First Aid certification is required

Job Duties & Responsibilities
Direct Services with youth ages 14-24
Maintain a caseload and individual student files as mandated by Workforce Investment Opportunity Act (WIOA).
Ensure all required documents are accurately completed and in student files
Input and Maintain OSMIS state electronic records in accordance with WIOA requirements for all WIOA participants.
Grow Detroit Young Talent SYE site recruiter, job developer and facilitator.
Recruitment, registration and retention of youth for all program participants (GDYT, Teen Achievers, Youth in Government, In-School and After School sites).
Assist with planning and execution of program events and activities
Serve as lead facilitator for Teen Achievers programming at 2-3 school sites, Thursday evenings, and some Saturdays.
Serve as a liaison between the YMCA, family, and community. Attend partnership meetings and other community meeting
Conduct interviews with work site supervisors, parent/guardian.
