Limited time only- $10,000 SIGN ON AVAILABLE FOR FULL-TIME
The Physical Therapist is responsible for providing physical therapy services to patients in their home in accordance with a written Plan of Care approved by a physician. The Physical Therapist makes an initial evaluation and plans with patient, and physician for on-going physical therapy services. Initiates, reviews and implements the PT Care Plan for each patient based on the assessment and prescribed Plan of Care and revises when indicated.

2 or more years of Physical Therapy experience
Exceptional customer service and computer skills
State of Maine PT License
Basic Life Support Certification
Valid driver's license
