Description
Young House Family Services is looking to fill a part-time position to provide support for the IHH Program in delivery of the program's services to eligible members. Duties include:
- Complete all intake paperwork with new members.
- Make referrals to community, social support, and recovery and resiliency services.
- Facilitate the development of comprehensive and crisis plans.
- Participate in hospital/out of home placement discharge planning.
- Monitor for potential crisis escalation/need for intervention.
- Follow up phone calls and face to face visits with members/families after discharge for hospital/out of home placement.
- Enter data in electronic medical record within established time frames.
- Monitor member records to identify and address gaps in service
Requirements
- Bachelor’s degree required. Related experience preferred.
- Must have valid driver’s license, reliable transportation, auto insurance, & pass all background checks.
