LCSW perform patients visits via telehealth remotely from their home. This is a full-time salary-based position, with the opportunity to receive a monthly bonus.
The Following Shift Is Available
- 8-hour shift (8:00am-5:00pm; Monday-Friday)
- Complete psychological assessments, social history and diagnose using ICD-10 codes of new patients for SC House Calls.
- Provide psychotherapy via telehealth for in house referred patients.
- Confirm type of insurance coverage prior to providing behavioral health services.
- Bill services provided through EMR and chronic care management system (CCM).
- Communicate with other providers within the interdisciplinary team to ensure the best care for patient.
- Align patients with community resources to create supportive relationships within their own environment.
- Complete documentation within EMR system to outline services provided (i.e. treatment plan).
- Communicate with patient's family members to provide necessary resources as needed.
- Attend virtual meetings as needed to staff and coordinate treatment for identified patients.
- Meet and exceed productivity goals and expectations for assigned caseload.

- Must be a License Clinical Social Work
- A minimum of one year of experience in a social work setting preferred
- Previous experience in social work and/or counseling setting is preferred
- Must be able to work flexible hours and travel between offices, facilities, and other places as needed.
- Ability to utilize electronic documentation required.
