Job Summary
The Youth Peace Center of Roseland (YPC) is seeking a dedicated and compassionate Case Manager to support individuals and families impacted by violence. The Case Manager will play a critical role in providing direct services, coordinating care, and fostering partnerships within the community. This position is essential to the success of our Violence Prevention Program, which aims to reduce violence and promote safety through education, advocacy, and intervention.
Key Responsibilities
- Case Management:
- Conduct comprehensive assessments of clients' needs and develop individualized service plans.
- Provide one-on-one support to individuals and families, addressing issues such as trauma, housing instability, education, employment, and personal safety.
- Act as a liaison between clients and service providers, ensuring timely access to resources and support.
- Advocacy and Support:
- Advocate for clients within systems such as law enforcement, legal services, education, and healthcare.
- Assist clients in navigating challenges related to violence prevention, conflict resolution, and non-violent communication.
- Provide crisis intervention and emotional support, including on-call availability during emergencies.
- Program Coordination:
- Work collaboratively with the Program Manager and other team members to implement violence prevention strategies.
- Facilitate workshops, group sessions, and community outreach events focused on education and skill-building.
- Maintain detailed records of client interactions, progress, and outcomes in accordance with program requirements.
- Monitoring and Reporting:
- Track and report on program outcomes, including client engagement, referral success, and overall impact.
- Participate in program evaluations and recommend improvements to enhance effectiveness.
- Community Engagement:
- Build relationships with community organizations, schools, and service providers to create a strong network of support for clients.
- Represent YPCR in community meetings and advocacy events to promote violence prevention and intervention efforts.
Qualifications
- Education:
- Bachelor’s degree in social work, counseling, psychology, or a related field (Master’s preferred).
- Experience:
- Minimum of 2 years of experience in case management, social services, or a related field, preferably working with youth or individuals impacted by violence.
- Skills:
- Strong interpersonal, communication, and organizational skills.
- Ability to work with individuals from diverse backgrounds and in sensitive situations.
- Knowledge of trauma-informed care practices and community resources.
- Proficiency in Microsoft Office Suite and case management software (preferred).
- Requirements:
- Must pass a background check.
- Flexible schedule with availability for on-call responsibilities as needed.
Job Type: Full-time
Pay: $40,000.00 - $45,000.00 per year
