Nurse Practitioner (Part-Time, 24 Hours/Week)

Position Overview: The HealthBar Nurse Practitioners work directly with Healthcare Partnership Program (HPP) employees and families to advance HealthBar's vision of improving community health and well-being. This role involves providing a range of services, from chronic disease management and preventative health to minor injury and illness care, primarily within on-site practice environments.

  • Providing advanced clinical support to patients, including acute and episodic care, primary preventative care, and chronic disease management.
  • Collaborating with HPP nurses on data analysis to help drive meaningful change.
  • Collaborating with our provider team to establish clinical pathways and treatment protocols for patients.
  • Conducting telehealth visits for HPP patients.
  • Coordinating care with the patient’s outside medical network.
  • Acting as a consultant and advocate in clinical settings, maintaining and promoting standards of care.

Qualifications and Requirements:

  1. Licensure: Current licensure as a Registered Nurse (RN) in the state of Michigan.
  2. Education: Graduation from an accredited nursing program (Bachelor's degree in Nursing).
  3. Certification: Active and unrestricted FNP National Board Certification from one of the following organizations:

    American Academy of Nurse Practitioners or American Nurses Credentialing Center

  4. Active and unrestricted State of Michigan-issued Registered Nurse and Nurse Practitioner Specialty Certification License.

  5. Department of Transportation (DOT) Examiner Certification (preferred, training provided if needed).

  6. Basic Life Support (BLS) Certification from the American Heart Association or American Red Cross within 90 days of employment.

  7. Minimum of 2 years providing care as a nurse in applicable areas such as med-surg, ER, ICU, acute care, public health, school nursing, or outpatient offices (preferred).
