Reports to: Director of Nursing
FLSA Code: Non-exempt
Hours: PRN As NeededPosition Summary:Responsible for the organization, throughput, census management, and ongoing performance improvement of patient placement. Responsible for management and placement of all admissions (elective and emergent) and transfers in accordance with the clinical standards and guidelines. Advises the Case Management team, Shift Supervisors, and Transitional Unit staff on day-to-day operations and throughput effectiveness and efficiency. Serves as the liaison agent between all departments.
Position Accountabilities:
  • Responsible for ensuring the patient is assigned to the most appropriate bed available using criteria for admission, and advising if the patient is a candidate for observation, admission, and or/an alternative level of care.
  • 24-hour responsibilities of Capacity Management.
  • Applies admission criteria accurately for all admissions to ensure the patients being admitted meet medical necessity for the status and level of care being ordered.
  • Consults and communicates effectively with the ordering physician to review admission and observation status when indicated.
  • Leads daily Capacity Management planning meetings, communicates, and collaborates with case managers and shift supervisors to expedite discharges and arrange proper patient placement for 23 hr observation and pending admissions.
  • Demonstrates knowledge around disease entities, around epidemiological issues and around circumstances that may change where a patient can be placed.
  • Collaborates with nurse managers and housekeeping to assure that nursing and support staff are expediting discharges and bed turnover.
  • Acts as a resource/advisor to physicians and nursing staff issues that may affect resource utilization and reimbursement.
  • Recommends new concepts, approaches, and procedures in the placement or discharge of patients to facilitate throughput.
  • Communicate with the emergency department and surgical services frequently to prevent a backup of admissions and to put into place procedures that might stop a backlog.
  • Directs and assigns transitional staff to areas of staffing need.
  • Communicates with medical staff, administration, and departments on a daily basis regarding capacity management.
  • Consults in weekly multidisciplinary patient care conference.
  • Uses critical thinking skills to analyze and implement JCAHO and Medicare standards and functions relevant to throughput and observation to maintain compliance as a Critical Access Hospital.
  • Integrates the programs of the Hospital and the assigned area to achieve the goals of the organization.
  • Maintains positive behaviors, attitudes, and commitment to interpersonal service toward patients, visitors and the care team.
  • Maintains competency and enhances professional growth and development through continuing education, independent learning, conference and seminars.
  • Assumes additional administrative and supervisory responsibilities are requested.
  • Responsible for abiding by and adhering to all HDH Corporate Compliance Program initiatives and standards.
  • Responsible for compliance with HDH safety standards, policies and procedures.

Position Qualifications:Education – Required: Graduate of accredited school of Professional Nursing. Current registration with the Ohio State Board of Nursing.
Minimum Experience: 5 years clinical experience
