STAR Services is looking to expand our team of person-centered planning facilitators. Our facilitators help people identify their strengths, preferences, and desires as they define the direction of their life, promoting positive control in their lives. Facilitators work in the Twin Cities metro area, travel throughout Minnesota, and also work remotely.

Qualifications:

· Completed Person-Centered Thinking (2-day training)

· Person-Centered Planning experience with PATH, MAPS, Liberty Plan, and other planning methods

· 3 years of experience in the disability services field

· Strong understanding of person-centered planning principles and practices

· Exceptional listener

· Great communicator

· Strong organizational skills and attention to detail

· Commitment to empowering others

· Creative, out of the box thinker

· Good balance of being flexible with people and teams while also being organized and helping teams take action

· Ability to work one-on-one with a person and also facilitate group sessions with their support team

· Maintain confidentiality and ensure compliance with data privacy standards

· Work is remote and also at various locations throughout Minnesota

· Ability to work independently and as part of a team of colleagues

· Must be able to pass a DHS background study

· Valid driver’s license and vehicle

· Competent in Microsoft Office Suite

Pay: From $50,000.00 per year
