Position Summary : The Licensed Practical Nurse or Licensed Vocational nurse as defined by state, reports to the Director of Nursing, and is to be clinically competent; delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacts with patient, significant others, and other health team members while maintaining standards of professional nursing. Limitation of assessment as designated by scope and state.

The LPN has the primary responsibility for providing medical services, counseling, supervision and program plan implementation to clients during scheduled shifts within known scope of practice. Direct client care involves the following duties:

Job Duties and Responsibilities:

· Utilize the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of the patient's care, as evidenced by client/staff observation.

· Assess the patient; collect and document the patient's current health status and medical history. Complete and/or review Admission Data Record as defined by policy. Make accurate and ongoing assessment of patient's status and respond appropriately.

· Plan/Implement or inter input; set priorities for nursing action in a logical sequence according to patient's needs and formulate a plan of care by the appropriate selection of Nurse Practice Standards. The Plan of Care is initiated on patient admission based on all aspects of the initial assessment as performed by designee within scope. Plan includes physiological, psychosocial, and environmental factors: patient/significant other education and patient discharge planning.

· Evaluate; nurse can input assessment and revise plan of care based on new data and patient's condition.

· Document accurate and ongoing assessment of patient status. Document patient care which reflects nursing intervention, patient response to care provided, patient needs, problems, capabilities, limitations, and progress toward goals. Documentation includes evidence of appropriate patient/significant other teaching, and the understanding of these instructions is noted in the medical record.

· Communicate with the physician regarding patient needs, the nursing assessments, and recommendations on a daily basis, and as needed, as evidenced by client feedback and Operations Coordinator observation. Collaborate with other nurses and health team members to ensure the patient's well being as evidenced by staff/client feedback.

· Teach patient/significant other appropriate health information in a timely manner and share written informational material as evidenced by client feedback and Operations Coordinator observation. Perform direct patient care, utilizing established procedures, policies, and standards as evidenced by client/staff feedback and Operations Coordinator observation.

· Administer medications according to policy and procedure.

· Administer IV's according to policy and procedure if certified and within scope of practice as designated by state

· Observe and document patient's response to pertinent medications.

· Demonstrate ability and appropriate technical skills when administering treatments and procedures in accordance with physician's orders and policy and procedure (done within a timely manner).

· Demonstrate necessary skills and knowledge to provide care for patients according to policy and procedure.

· Demonstrate necessary skills and knowledge to provide care for patients according to division/unit specific competencies.

· Demonstrate ability to handle emergency situations in a prompt, precise, and professional manner. Provide personal patient care to provide comfort and well-being to the patient, acknowledging physiological and psychological needs.

· Perform indirect patient care, utilizing established procedures, policies, and standards as evidenced by client/staff feedback and Operations Coordinator observation.

Give a thorough, concise report at change of shift. Communicate pertinent information to the appropriate person in a timely manner.

· Demonstrate accurate and timely documentation according to form guidelines.

· Accurately transcribe physician orders according to policy and procedure.

· Maintain a clean, neat, and safe environment for patient and staff according to policy and procedure as evidenced by staff/client feedback and Operations Coordinator observation.

· Ensure that supplies and equipment necessary for patient care are stored in an organized and efficient manner. Demonstrate correct and safe technique in the use of equipment according to specific product information. Follow appropriate procedure for obtaining and returning or cleaning/disposing of equipment and supplies. Participate in economical utilization of supplies and ensure that equipment is maintained in a clean and safe manner.

· Adhere to Administrative System/Nursing Service policies as evidenced by Director of Nursing

· Do not incur excessive overtime. Remain conscientious in regard to personal hygiene and comply with specific division/unit regulations.

· Comply with specific System/division/unit policy and procedure concerning safety.

· 100% attendance to mandatory skill updates yearly required.

· Attend a minimum of four staff meetings and read and sign all minutes of meetings not attended.

Teamwork:

· Assist in the orientation/training of new Team Members.

· Consistently work in a positive and cooperative manner with fellow Team Members.

· Assist other Team Members in the performance of their assignments.

· Seek out opportunities to help rather than waiting to be asked.

· Consider the impact of your actions on Team Members throughout the System.

· Recognize the need for variations in staffing and volunteer to fill open shifts.

· Take direction and initiate actions (cross/additional training) that will allow the assumption of cross-functional duties to ensure seamless patient care.

· Demonstrate flexibility to perform duties wherever volume deems it necessary within the System.

PROBLEM SOLVING

· Demonstrate sound judgment by taking appropriate actions regarding questionable findings or concerns.

· Investigate and follow through on unusual orders or requests for service or information.

· Follow proper reporting procedures for actual or potential accidents and/or incidents so follow-up and/or prevention can occur.

· Record/report the need for service maintenance or repair of equipment and remove any faulty equipment from service.

· Contribute to the achievement of Performance Improvement and participate in

· Demonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate books and manuals.

· Comply with the System's infection control policies and procedures including Body Substance Isolation (BSI) and the Exposure Control Plan which reflects OSHA Bloodborne Pathogens Standard to ensure a safe working environment for self and others.

· Ensure that the System is operated in an efficient and cost effective manner by adhering to System and departmental attendance policies as documented on the Employee Attendance Record.

· Demonstrate adherence and support of System-Wide Service Standards as evidence by observation and client feedback.

· Demonstrate adherence to System/Department Complaint Management Standard.

· Demonstrate adherence to System/Department Dress Code Standard.

· Demonstrate adherence to System/Department Safe Practices Standards.
