6 Roses Licensed Practicing Nurse (LPN) Job Description
SUMMARY
$500 Sign on Bonus!! The Registered Nurse (RN), Licensed Practicing Nurse (LPN) for 6 Roses Home Healthcare performs two important tasks: Start of Care (SOC) and Recertification (Recert) both are which explained below. Effective communication and documentation is a must for this position.
ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING:
The RN/LPN works with the Office Manager and the scheduler to obtain information about opening new client cases. The scheduler or the office manager will inform you about a new case and complete a welcome call. Once the welcome call is completed the scheduler will send the information to the RN/LPN and she will contact the client and set up a time to open the case. The RN/ LPN will take the client folder to the client and obtain all the information by completing all the documentation start of care packets. Gaining some basic vitals (temp, blood pressure pulse etc.) of the client will also be obtained at the time.
The other part of the job is every 60 days a recertification is done on all clients. The LPN will call and set-up a time with the client to complete all of the documentation and submit it to the office.
Communication and documentation are very important and essential between the nurse and the office staff. This needs to be done every time a client case is opened or a recertification is completed. Other duties may be assigned.
EDUCATION AND/OR EXPERIENCE
Registered Nurse/Licensed Practical Nurse in state of practice. Graduate of an accredited practical nurse program. At least one year of nursing experience. Community/home health
