Responsibilities:
- Conduct assessments and develop personalized treatment plans for patients based on their specific needs and goals.
- Implement therapeutic exercises, manual therapy techniques, and modalities to improve patients' physical well-being.
- Monitor and document patients' progress, adjusting treatment plans as necessary to achieve optimal outcomes.
- Educate patients and their families on exercises and techniques for continued improvement outside of therapy sessions.
- Collaborate with other healthcare professionals to ensure comprehensive care for patients.
Requirements:
- Bachelor's, Master's, Doctorate's degree in Physical Therapy from an accredited program.
- State licensure as a Physical Therapist.
- Experience in SNFs, ALFs, Memory Care recommended
- Strong interpersonal skills and the ability to communicate effectively with patients and colleagues.
- Ability to work independently as well as part of a multidisciplinary team.
- Knowledge of current trends and best practices in physical therapy.
