Children's Health Network is Hiring a Care Manager Near Massena, NY

FLEX SCHEDULE - STRONG SUPPORTER OF WORK-LIFE BALANCE

Pediatric Health Homes Care Management Agency is seeking an experienced, responsible, motivated professional Care Manager to provide advocacy and assistance to families with medically fragile / physically disabled children.

Individual will be responsible for assisting and supporting families with securing necessary services for their medically fragile / physically disabled children, while maintaining numerous Health Home programmatic requirements and compliance.

Bachelor's Degree REQUIRED

Care Management experience STRONGLY PREFERRED

Mandatory Background Check and SCR Clearance conducted upon hire.

Work from home with mandatory field-based requirements. Valid Driver's License and reliable vehicle a must. Travel throughout St. Lawrence and Franklin counties required.

PART TIME OPPORTUNITY: 8-14 clients [approximately 20-25 hours per week]
