We are currently seeking a Family Service Coordinator (FSC) to implement the rehabilitation activities outlined in the Clients IRP. This position is supervised by the Rehabilitation Specialist and provides direct services to clients to include weekly sessions, individual / family, track progress of IRP goals, provide community referrals, meet with clients both in the community and office. PRP counselor assist clients in meeting immediate needs such as food, clothing, housing, employment and substance abuse treatment.

Must have personal transportation.
