Health Care of South Florida is a skilled nursing home health agency established since 2005.

We are hiring for an Admissions/Intake Coordinator for HMO, Medicare, Medicaid and Private pay. This position will be at our Pembroke Pines location.

The applicant must be a detail oriented person, dependable with excellent organization and communication skills. Must be able to handle multiple calls (high volume of calls daily).

Some of the responsibilities include but are not limited to:

· Taking referrals for new patients as needed, this includes following up with hospitals for documentation, communication with Case Manager hospitals and referral sources.

· Verify insurance in Medicare portal, Availity and Allscripts.

· Welcome call to patients after intake process.

· Staff and schedule visits for caregivers to go to patients and create schedules in Kinnser. Maintain open communication with patients about visits and check compliance of visits with caregivers.

· Communicate with hospitals about discharges and notify to all involved.

· Work as a team with Medicare Case Manager and PCR Coordinator for a smooth process on Medicare billing.

· Upload documentation required in the patient file in Kinnser (MD orders, hospital documents).

· Maintain documentation pertaining their job (Hospital transfers, Intake count, non admissions).

Requirements:

· HOME HEALTH experience (MUST)

· LPN (Strongly Preferred but NOT A MUST)

· Bilingual. English and Spanish (MUST)

· Kinnser system experience (PLUS)

. Allscripts/ Extended care referral portal knowledge (MUST)

· Insurance verification (CMS, Availity, Allscripts, NaviHealth)

· Ability to interpret clinical information

· Ability to collect all relevant/necessary information

· Good writing and communication skills

· Experience communicating with doctors and nurses taking orders and referrals

Job Types: Full-time, Contract
