Job Overview:

Responsibilities:
- Administer medications and treatments as prescribed by physicians
- Monitor and record vital signs, such as blood pressure, temperature, and pulse
- Assist with catheterization and tube feeding procedures
- Provide wound care and dressings
- Collaborate with registered nurses and physicians to develop and implement patient care plans
- Document patient information accurately and maintain up-to-date medical records
- Educate patients and their families on proper healthcare procedures and disease management
- Ensure compliance with all applicable laws, regulations, and policies

Qualifications:
- Valid LPN license in the state of [state]
- Previous experience in home care, assisted living, or a similar healthcare setting is preferred
- Proficiency in performing vital signs monitoring and basic medical procedures
- Knowledge of ICD-10 coding system for accurate documentation
- Familiarity with electronic medical records (EMR) systems
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members
- Ability to lift and move patients when necessary
