Full job description

We are looking for Qualified Mental Health Professionals (QMHP A-C) to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. This position involves the provision of an array of counseling and educational services within the home and community setting. QMHPs are responsible for all client related documentation and reports as well as the implementation of treatment goals and objectives. Provide services in the form of direct involvement with clients and/or their family members. Position reports directly to DOS. QMHP certification is a must.

Responsibilities

  • This position involves the provision of an array of counseling and educational services within the home and community setting.
  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
  • Handle case assignments, review case progress and determine case closure
  • Help clients achieve wellness and autonomy
  • Facilitate multiple care aspects (case coordination, information sharing, etc)
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
  • Provide individual, casework services in accordance with each client’s Individualized Service Plan
  • Provide case management ensuring the continuity of services for each client by managing all aspects of the ISP and coordinating service delivery among all treatment components to meet the needs of the client.
  • Responsible for all client related documentation, progress notes and reports as well as the implementation of treatment goals and objectives.
  • Promote quality and cost-effective interventions and outcomes
  • Adhere to professional standards as outlined by protocols, rules and regulations
  • Continually assess the client's needs and coordination of resources to meet these needs
  • Maintain a high degree of professionalism in the community, with clients, and with referring agencies seeking to build and maintain positive relationships.
  • This position involves the provision of an array of counseling and educational services within the home and community setting.
