Next Step Program Intensive Case Manager: Reports to Executive Director of The Chamber Foundation, Inc.

Primary Functions:

Work directly with program participants in a professional and respectful manner to provide comprehensive job seeking and job retention services as required in the annual DCFS Workforce Connections Contract.

Detailed Duties:

1. To market the Workforce Connections Program to community service organizations and potential employers to promote the benefits of the program throughout the local social service and employment communities.

2. Follow enrollment policy for step-by-step process on how to enroll qualified individuals.

3. To create a participant file for each new enrollee by having an enrollee complete the Participant Profile, Participant Agreement, Signed Confidentiality Release and waivers, conducting a thorough job-readiness interview, obtaining personal history and finally completing participant SMART Goals.

4. Develop and implement ongoing workshop series for program participants that includes but is not limited to; job readiness training, soft skills development, budgeting and financial literacy, conflict resolution and round table discussions. Identify training opportunities for program participants and work in collaboration with DC BOCES, DCC and other training providers.

5. Respond promptly to referrals from the DCFS. The case manager will contact the candidate by phone or in writing no less than four times to schedule an informational meeting. All efforts made to contact participant must be marked in Civicore Database.

6. After the initial meeting, the case manager will contact the client at least once every week by phone or in person for the next six (6) months.

7. Assist the client in assessing supportive or transitional services available within the community or by DCFS in order to enable him/her to remain employed.

8. The case manager will meet with or conduct a phone interview with the client’s job supervisor, or whomever the employer designates for this purpose, within five (5) business days of the candidates employment start date or program enrollment. All meetings and phone calls with the employer will be scheduled at the employer’s convenience.

9. The case manager will contact the employer not less than two times per month and record all such contacts in the case file and case notes. The purpose of such contacts is to address any specific workplace issues and to obtain feedback on the client’s overall progress. More frequent employer contacts will be made if it is deemed beneficial by the employer to the client’s progress. The case manager will contact the client immediately if notified by the employer or the DCFS that the client has failed to come to work.

10. The case manager will capture case notes in Civicore Database for each meeting or phone call to the employer and to the client. The case manager will report the client’s progress to the DCFS Employment division on a monthly basis. Each monthly report will be provided to the DCFS prior to the fifth working day of the next month.

11. To ensure the confidentiality of all written information, oral communications and personal correspondence of program participants at all times.

12. Conduct home visits and home based meetings, as well as other community visits with clients on a regular basis (i.e. school meetings, legal proceedings, appointments where additional support is needed)

13. The case manager will provide the DCFS with an evaluation of each client’s progress on a Workforce Connections Participant Evaluation form at the end of every three months of continuous employment. The evaluation form may be completed by either the employer’s representative or the case manager after interviewing the employer.

14. The case manager will act as a liaison with the DCFS Employment worker regarding the individual client’s progress on an as needed basis. The case manager will assist the client in assessing supportive or transitional services available within the community or by DCFS in order to enable him/her to remain employed.

15. The case manager will report to the Workforce Program Manager on regular basis to update all information relating to the program and client progress. If the Workforce Program Manager is unavailable and a crisis occurs, the mentor will report this information to the President of the Chamber.

16. Assist with monthly and quarterly reports

17. The case manager will work cooperatively with the Chamber staff and staff of any other agency or organization participating in this program. The case manager will provide assistance and support to employers as it relates to the successful retention of the mentee, and to ensure customer satisfaction. The case manager will provide quality customer-driven service to all program participants, including employers, mentees, and personnel of participating agencies and organizations.

18. The case manager will attend all scheduled mentoring meetings unless excused in advance by the Workforce Program Manager. Attend informational, community, and Chamber committee meetings and events as requested.

19. The case manager will submit mileage and expense vouchers on the appropriate form by the last day of the month. Any expenditures outside of regular mileage must be approved in advance. Under no circumstances should Chamber or personal funds be expended on program participants outside the scope of the mentoring contract.

Other Duties: As an employee of The Chamber Foundation, the case manager will assist with Chamber events such as monthly contact breakfast, business after hours, ribbon cuttings, other events and other duties as assigned.

Performance: The case manager will provide services to the program participants with the goal of successful, long-term employment and development of the individual.

Job Type: Full-time

Pay: $25.00 - $26.50 per hour

Expected hours: 35 per week
