Moving Forward Towards Independence is Hiring a Case Manager Near Napa, CA

Moving Forward Towards Independence, a unique and dynamic residential/supported living program for adults with cognitive disabilities in Napa is seeking a Full Time or Part-Time Case Manager.

Purpose: to support our residents to live as independently as possible in their own homes.

Primary Responsibilities:

-Responsible for assessing residents interested in living independently and developing
Individual Support Plans, regularly monitoring goal progress, and updating resident records.

-Provide instruction and support around all areas of daily living, including, but not limited to:
-Meal preparation, shopping, hygiene, personal care, budgeting, safety awareness, and maintaining a clean and healthy living environment.

-Maintain regular, ongoing contact with the resident's family or guardian with regards to goals and/or emergencies.

-Assist residents in coordination of all medical / psychological appointments.

QUALIFICATIONS:

*A clean driving record and clear criminal background check required*

Bachelor's Degree (in Special Education, Counseling or equivalent preferred); and/or at least two years experience working for adults with mild developmental, psychiatric, Spectrum or learning disabilities, preferably in an independent/supported living environment in a case management/teaching role.

Must be willing to work flexible hours, (Example: Tuesday-Saturday or Sunday-Thursday from 11am-7pm or 12pm - 8pm on some days) to meet resident needs.

The ideal candidate must possess initiative, strong communication skills, excellent interpersonal/teaching skills and be able to work with others as a team.
