Epiphany Care Homes was created for seniors with dementia, Alzheimer’s, and mood disorders. We focus on providing 24-hour compassionate assisted living support in a residential home environment with only 5 to 7 residents per each of our three locations.

We currently have an opening for two part-time Activities Coordinator to join our team! The Position is part-time, with shifts 2PM- 8PM during the week, and every other weekend. The position is available immediately!

POSITION SUMMARY: Responsible for planning, directing, implementing activities program for residents. Assures that activities occur promptly, required documentation is completed, and that the department complies with applicable Federal and State regulations.

JOB/DUTIES RESPONSIBILITIES:

• Work with the Site Lead and Nursing Care Team to develop, design, implement, supervise and evaluate an activity program every month that meets the needs of the residents at Epiphany Care Homes.
• Direct Activity participation in the care planning process.
• Plan, organize, and implement a variety of recreational programs and activities for senior residents in assisted living.
• Prepares a monthly calendar of activities that is posted in advance of the 1st of the month, to ensure a diverse range of activities that cater to residents' interests and abilities.

This will include large and small group activities and opportunities for individual activities.

• Arranges for the supplies to complete Activities, including at times transportation. Manage activity budgets, resources, and supplies effectively.
• Coordinate outings, social events, and group activities to promote socialization and mental stimulation.
• Participates in meetings regularly with the administration team and our CNA care team to ensure that resident needs are being met. Collaborate with staff, residents, and families to assess individual needs and preferences to tailor activities accordingly.
• Screens, monitors, and schedules all volunteers providing service at Epiphany Care Homes.
• Participates in and provides in-service as requested. Also attends continuing education as appropriate to remain up to date with aspects of resident care.
• Responds to call lights and alarms when in the area of notice.
• Other duties as assigned by Administrator. Position responsibilities may change as needed by the organization.

REQUIREMENTS:

Requirements:

• HS diploma
• Driver’s license and driving record
• Computer software skills, English language, communication
• Experience in planning, implementing and organizing events and activities
• Experience working with elderly and vulnerable adult populations

Preferred Requirements:

• Therapeutic recreational specialist or other therapeutic license/experience
• CPR certification
• First Aid certification
