The Elementary School Counselor is responsible for providing a comprehensive school counseling program that is preventative in design, developmental in implementation, and supports St. Mary’s students in the areas of academic achievement, individual student planning, and personal and social development. The incumbent serves as a liaison between students, educators, families and community partners. Incumbent reports to the Director of Student and Family Services.- City Connects Program Coordinator - Work with faculty and family members from the elementary division to identify and respond to the strengths and personal developmental needs of every child in grades PK-5- Refer students to the Child Study Team if further evaluation and/or consultation is necessary- Attend all meetings regarding PK-5 students- Provide consultation and support for families and faculty members to meet the needs of identified students- Connect students to a range of prevention, early intervention, and intensive service programs that address the physical, emotional, cognitive and social development of each student- Provide mental health, behavioral and academic support to students in grades PK-5- Consult with school leadership regarding student behavior concerns and develop and facilitate the implementation of behavior plans as needed- Consult with parents and faculty regarding student accommodations per IEP and 504 plans- Coordinate related services for students (speech, OT)- Facilitate annual reviews and re-evaluations with home districts for students with IEPs- Assist parents in the referral process for special education services- Collaborate with elementary assistant principal and reading specialist to analyze student data and implement supports based on analysis - Facilitate lessons in all classrooms during SEL block, utilizing PK-5 scope and sequence- Conduct individual and small group meetings with all pk-5 students - Supervise lunch daily to build rapport with students - Monitor Attendance and implement school-wide outreach policy and procedures- Support the school-based Cartwheel telehealth process for students and families- Maintain frequent communication with parents- Additional duties as assigned
