Position: HOCTIL Independent Living (IL) Specialist

Service Delivery Area: Central – Bell and Coryell

Employment Status: Full-time, non-exempt employee with benefits

HOCTIL promotes a philosophy of independent living; including consumer control, peer support, self-help, self-determination, equal access, and individual and systems advocacy, in order to maximize opportunities for individuals with disabilities and/or chronic illness and the integration and equal access to participate in the community.

General Description: The IL Specialist is responsible for assisting persons with disabilities and/or chronic illness receive appropriate services, which will enable them to live and work more independently within their community.

Essential Tasks:

1. Responsible for the development, implementation, and expansion of HOCTIL services in the core county service area.

2. Responsible for conducting intake (home visits are mandatory) and assessments of all consumers seeking independent living services.

3. Responsible for promoting and representing the organization to potential clients/consumers, agencies, general public, and other disability organizations.

4. Receive and respond appropriately to I&R requests, by phone, written request, or in person, provide information and make appropriate referrals in a timely manner.

5. Provide orientation to Independent Living Philosophy and IL services to new consumers and those who request individual services.

6. Provide the five core services: Information & Referral, Peer Support, Independent Living Skills Training, Individual and Systems Advocacy, and Transition Services.

7. Provide Ticket-To-Work services to consumers to include work plans for those individuals wanting to rejoin the workforce and assign their ticket.

8. Maintain full, accurate, detailed, and current documentation of evaluations, assessments, needs, progress, services, financial data, and all other categories of information required in each Consumer’s record.

9. Understands, subscribes to, and advocates the philosophy and values of independent living, culturally relevant education, empowerment, and community integration.

10. Maintains thorough knowledge of appropriate health and social services delivery systems and provides opportunity for consumers to make informed decisions.

11. Meets regularly with Consumers to discuss progress, problems, and plans.

Non-Essential Job Functions:

  • Other duties as assigned by Executive Director.
  • Attend trainings on an as-needed basis.
  • Expand Assistive Technology.

Supervisor of this position: Executive Director of HOCTIL

Requirements:

1. MinimumBachelors of Social Work Degree.

2. Knowledge of community services & related resources for persons with disabilities or chronic illness to live in their community.

3. Computer skills, interpersonal communication skills, and ability to work independently.

4. Ability to maintain a large number of consumer files.

5. Ability to handle stress and work under pressure.

6. Must understand commitment to the concepts and philosophy of independent living and self-determination of persons with disabilities.

7. Personal experience with a disability preferred.

8. Available and reliable transportation, as travel within the service area is required.

9. Valid Driver’s License & insurance.
