About Us:

We are a unique clinic aimed at improving the lives of seniors in their homes through a holistic, patient forward approach. We are looking for a team member who can assist with at home assessments of our patients and ensuring that their holistic needs are met.

About you:

We are seeking a registered nurse with at home case management experience. We serve patients in assisted living facilities and in surrounding rural areas. Individuals with experience serving seniors and a passion for serving underserved patients are a must. This is a remote position with field travel to Seattle, Snohomish and Tacoma for in person patient visits.

Pay:

We pay per hour of care management time $55-60/hr and a stipend for travel days (approximately 1-2 days per week). This includes patient visits, phone follow up, and work with referrals, scheduling or care. We are seeking a highly motivated self-starter with a passion for patient care and interest in helping us to provide the highest quality care available.

Abilities/Skills

• Comprehensive assessments of patients based on clinical judgment, identifyinng changes in member's health or psychosocial wellness, and triggers identified in the assessment.
• Develops and implements a case management plan in collaboration with the member, caregiver, physician and/or other appropriate healthcare professionals and member's support network to address the member needs and goals.
• Conducts face-to-face or home visits as required.
• Performs ongoing monitoring of the care plan to evaluate effectiveness, document interventions and goal achievement, and suggest changes accordingly.
• Maintains ongoing member case load for regular outreach and management.
• Promotes integration of services for members including behavioral health care and long term services and supports/home and community to enhance the continuity of care
• Facilitates interdisciplinary care team meetings and informal ICT collaboration.
• Uses motivational interviewing to educate, support and motivate change during member contacts.
• Assesses for barriers to care, provides care coordination and assistance to member to address concerns.
• Assigned cases with members who have complex medical conditions and medication regimens

Up to 50% local travel time required

Qualifications

- Valid RN license in WA

- Minimum of 5 years of nursing experience, preferably in home care or a related field

- Strong communication and interpersonal skills

- Ability to work independently and prioritize tasks effectively
