Job summary

We are looking for experienced and talented Physiotherapist or Occupational Therapist to join our senior management team at Ealing Community Partners.

Working as the Clinical Service Manager you will be responsible for the management and leadership of our multi-disciplinary Short Term Rehabilitation Team of over 30 staff.

The role will include ensuring the delivery of high quality, personalised and accessible services for users with rehabilitation potential, in line with commissioned service levels.

The post holder will be required to engage and empower clinical staff in the provision of care andto use clinical expertise to assess, develop and enhance the quality of service.

Main duties of the job

Leading and managing the multidisciplinary Short Term Rehabilitation Service for Ealing.

Responsibility for the day to day operational management and service delivery

Developing and monitor clinical outcome metrics for designated areas proposing and implementing improvements where they are required.

Acting as an ambassador for the service developing robust communication strategies that provide all stakeholders with the opportunity to be engaged in service developments and informed on progress.

Developing partnership opportunities and integrated working with all health and social care providers ensuring personalised quality care for patients.

Ensuring high standards of clinical practice through education and training to staff and students.

Participating in the transformation of the wider Ealing community rehabilitation services deliver the ICB / North West London vision.

Job responsibilities

Clinical Duties

  • Demonstrate clinical expertise by providing direct advanced clinical care to a range of patients with rehabilitation needs.
  • To be able to use advanced clinical skills and expert knowledge to carry out thorough assessments of physical and psycho-social needs in order to develop comprehensive care plans that aim to prevent and reduce hospital admissions.
  • Act as an expert clinical resource in the development of advanced practice relevant to rehabilitation management.
  • Maintain a relevant clinical caseload.
  • Work with clinical staff to facilitate the implementation of NICE and evidence-based care within the community rehabilitation service.
  • Facilitate the translation of national policy into local delivery and to provide an expert perspective in relation to the planning and delivery of rehabilitation across the care pathway.
  • Lead on the development of an integrated community rehabilitation services that proactively manage patients with rehabilitation potential that aims to prevent and reduce hospital admissions and promote patient health and wellbeing.
  • Positively influence change within clinical practice across the community rehabilitation service
  • Act as a role model by working alongside staff supporting them to achieve and maintain high standards of clinical practice.
  • Ensure involvement of the patient and their carers, in decision making to improve understanding of their health care needs promoting self-care as appropriate.
  • Be aware of the support services available for patients and carers from statutory, voluntary and ancillary agencies for patients with long-term conditions.
  • Promote and facilitate patient choice in the planning and delivery of healthcare services.
  • To be alert to the needs of vulnerable adults, identifying risk of possible harm and taking appropriate action as required.

Person Specification

Qualifications

Essential

  • Registered therapist with current HCPC registration
  • Relevant therapy degree or equivalent
  • Post-registration qualification and evidence of continuing CPD

Experience

Essential

  • Extensive post-registration clinical experience at a senior level
  • Experience of working in a community setting
  • Experience of undertaking complex assessments and providing care to people with reablement / rehabilitation potential
  • Experience in team management including performance management
  • Experience of initiating and implementing changes to services

Knowledge

Essential

  • Knowledge across a range of long-term conditions
  • Knowledge of implementing and monitoring service outcome measures
  • Knowledge of clinical governance processes, risk assessment etc

Abilities

Essential

  • Ability to communicate highly complex information both verbally and in writing
  • Resourceful and able to affect change through leadership and influence
  • Able to teach in formal and informal settings
