**Overview:**

We are seeking a dedicated Personal Care Assistant to join our team.

You will assist clients in and out of bed, excluding the use of mechanical lifting equipment unless trained and documented as competent.

2. Assist clients with baths, Personal Hygiene, grooming, toileting, hair & skin care including the use of walkers and wheelchairs, when applicable.

3. Assist clients with meal preparation and medication reminders when needed.

4. Record and report changes in the clients physical condition, behavior or appearance to the SUPERVISOR OR CASE COORDINATOR.

6. Essential homemaker tasks. (Light House-Keeping)

7. Documenting services delivered in accordance with Agency policies and procedures.

**Responsibilities:**

- Provide personalized care and support to patients under the supervision of healthcare professionals

- Assist patients with activities of daily living, including bathing, grooming, and feeding

- Monitor patient vital signs and report any changes to the medical team

- Ensure patient comfort and safety at all times

- Collaborate with healthcare providers to implement care plans

- Maintain accurate patient records and documentation

**Experience:**

- Prior experience in a home healthcare setting such as ,outpatient clinics, or inpatient facilities

- Familiarity with emergency medicine protocols and procedures

- Knowledge of geriatric care practices

- Experience as a caregiver is a plus
