ChristianaCare HomeHealth is seeking a Full Time Care Transitions Coordinator - Social Worker. The successful candidate will support Dover, Smyrna, Milford and the surrounding areas; work day shift hours 8:30am-5pm, and an occasional weekend day or holiday based upon departmental needs. Duties and Responsibilities: Participates in Case Conferences and the development of total plans of care with the multidisciplinary team. Provides clinical social evaluations and plan appropriate interventions based upon evaluating findings. Identifies patients appropriate for specialty programs and Telehealth. Completes home visits to clients and discussions evaluation findings with Senior Medical Social Worker Identifies appropriate community resource referrals to address the client's and the family's practical and environmental needs.
