Overview: PRN / TEMPORARY / FULL-TIME
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will play a crucial role in coordinating and managing patient care to ensure optimal outcomes. This position requires a strong background in hospital medicine, utilization management, and primary care.
Responsibilities:
- Conduct comprehensive assessments of patients' medical conditions and develop individualized care plans
- Collaborate with interdisciplinary teams to coordinate and manage patient care
- Monitor patients' progress and make necessary adjustments to the care plan
- Provide education and support to patients and their families regarding their medical conditions and treatment options
- Utilize medical records and other resources to ensure accurate documentation of patient information
- Coordinate with healthcare providers, insurance companies, and other stakeholders to facilitate timely and appropriate care
- Advocate for patients' needs and ensure they receive high-quality, patient-centered care
- Utilize knowledge of physiology, medical terminology, critical care, and patient care to effectively manage complex cases
Qualifications:
- Must send a detailed resume
- Background check
- TB shot required
- Must possess current license from the Indiana State Board of Nursing.
- Minimum of 3 years of experience in hospital medicine, utilization management, or primary care
- Strong knowledge of medical terminology, physiology, and critical care practices
- Excellent communication and interpersonal skills to effectively collaborate with interdisciplinary teams and advocate for patients
- Ability to analyze complex medical information and make informed decisions regarding patient care
- Proficient in utilizing electronic health record systems and other healthcare technology
