Personal Supports Case Manager (FT) - Fluency in American Sign Language.Job Description: COMMUNITY SUPPORT SERVICES For the DEAF, Inc. Fluent in American Sign Language.POSITION: Personal Supports. STATUS: Full-Time (40 hours). WHO IS A CASE MANAGER?
A Direct Support Professional who assists Deaf or Deaf-Blind Adults with Intellectual/Developmental Disabilities.
Assistance includes Home-based assistance with personal care, cleaning, laundry, cooking, medication management, and assistance with mail.
Community-based assistance with medical appointments, shopping, socialization, and activities/events.
Other duties as identified in the annual Person-Centered Plan (PCP) meeting.
Expectations
Excellent time management skills, well organized, team player, positive attitude, patient and respectful.
Annual physical and flu vaccinations are required.
Qualifications
Bachelor’s degree or High School Diploma with 3 years relevant experience.
Valid MD Driver’s License with 2 points or less.
Fluent in American Sign Language (ASL).
