Job Summary
- The case manager is accountable for collaborating with the Multidisciplinary Care Team to promote quality patient care. Facilitates efficient utilization of resources.
- Identifies patient care needs throughout the continuum of care.
- In coordination of care through collaborative practice – has excellent communication skills.
- Knowledgeable of the Health Care Continuum and today’s challenge of change.
- Staff is required to maintain a level of physical/mental health needed to perform their duties effectively.
- Adheres to the Nursing Code of Ethics, St. Mary’s Healthcare Center’s Mission, Service Excellence Standards and the standards outlined in the CHI “Ethics at Work” booklet.
Education and/or Experience
- AS RN Associates Degree required
Preferred Education
- Bachelors Degree
Required Experience
- 1-2 Years
Preferred Experience
- 3-5 Years
Licensure/Certification/Registration
- Required License/Certification/Registration
- Graduate from an accredited school of nursing with current Licensure as a Registered Nurse in the State of South Dakota.
- BLS
Preferred License/Certification/Registration
- Bachelor of Science Degree preferred, not required
Work Schedule
- Monday – Friday, 8:00 am-4:30 pm; 80 hours every 2 weeks
Pay
The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
- $29.50 - $44.25 per hour
