Shift : 8 : 00 AM - 4 : 30 PM

Experience :

• 12 months of Physical Therapy experience is required.
• 3 months of experience with PointClickCare is required.

Requirements :

• Candidates must have a license prior to placement start.
• This role may require floating to additional units and locations

Additional Details :

Can submit with a block scheduling request - The facility will always try and honor, but never guarantee.

Guaranteed Hours per Week : 40

Shift Requirements : M-F 8-530

On Call / Call-Back : N / A

Weekend / Holiday : PT’s rotate one Saturday every 6-8 weeks. (the Sat. they work would be given a day off during the week) Saturday only. No holiday.

Required Education : Master’s Degree in Physical Therapy.

Minimum Experience : Home Health experience, training working with the adult / geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis.

Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy.

Charting / Equipment : Laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication.

Population served includes adult and geriatric patients.
