Job Scope: This position functions to complete nursing assessments, identify needed supports, participate in discharge planning, skill building and healthcare coordination for Williams and Colbert Class Members transitioning out of nursing facilities and into independent living situations. Works as a member of an interdisciplinary team and is responsible for healthcare coordination for a caseload of clients.
Clinical Duties
  • Meet with clients in their homes or nursing facilities to assess needs and develop treatment plan based on individual’s needs. Caseload location will primarily be in Lake County, though will vary based on program needs.
  • Review health records, obtain health history, complete in person nursing screenings and monitor health and psychiatric conditions.
  • Review comprehensive program intake assessment and provide feedback and medical recommendations for individuals transitioning out of long-term care settings.
  • Provide education and skill-building to prepare clients for transition from nursing facility to independent living. Examples include diabetes self-management, medication and symptom management, independent blood pressure monitoring and safe administration of anti-hypertensive medication.
  • Complete established program requirements, including RN screenings, follow up visits, post-discharge visit, post-emergency room visit, and post-hospitalization follow ups within grant’s required timeline.
  • Provide medication education and promptly respond to clients seeking assistance with medication side effects, medication questions, needed medication refills, or any medical or psych related issue.
  • Participate in the Comprehensive Transition Program care plan meetings, internal staffing, and case consult clinical calls as needed.
  • Work with interdisciplinary team to develop safety plans for clients and respond to medical and mental health crises when necessary.
  • Meet or exceed productivity expectations.
  • Schedule appointments and coordinate care, obtain referrals, and communicate information to physicians, pharmacies, and service team members.
  • Participate in service plan development and discharge planning for individuals transitioning out of nursing facilities and to community-based independent living.
  • Communicate clearly with interdisciplinary team, Class Members, outside supports, and other providers to ensure continuity of care.
  • Report any significant medical or clinical events to supervisor and treatment team.
  • Complete all clinical documentation and required reports in an accurate and timely manner.
  • Other duties as assigned.
Administrative Duties

  • Participate in orientations and trainings as required by grant and KYC.
Travel Requirements
  • The employee is required to travel by automobile to and from various KYC office locations on a weekly basis, as well as frequent client visits at their homes, area hospitals and nursing home facilities in the Lake County area, and occasional meetings/trainings in and around the Chicagoland area.
Position Requirements
  • Registered Nurse with current Illinois license
  • BSN degree preferred; AAS degree with additional experience considered
  • Preferred experience for individuals with chronic, serious mental illness
  • Preferred experience in community or nursing facility settings
  • Valid Illinois driver’s license, auto insurance, and access to reliable vehicle
  • Basic technology, smartphone, and computer proficiency; knowledge of web-based team communication tools (ex. Microsoft Teams), electronic health records, telehealth programs, and internet skills
